Dial things up with a custom macro-based nutrition plan.

In order to get started on a custom, macro-based nutrition plan, I need to first learn about your current eating habits. To provide this information in the most accurate way, you need to track your food intake for one week before I can design a comprehensive plan that will work. The app I recommend using is MyFitnessPalIt is the most common and comprehensive food logging app out there.

When tracking it is essential you maintain “normal eating habits” to ensure accuracy. Do not worry about calories, food choices, or whether you are "doing things right". I'm simply looking for common eating habits & quantities your body is used to. The most important piece is to be honest and consistent with what you typically eat. I can promise you, there is zero judgement from me. I am not surprised by anything! I will not design a plan for you without this information.

Once you have one full week of food logged, please enter your information below. This will prompt me to reach out and schedule your initial consultation.

Step 1: Tell me about yourself.

In this comprehensive client information sheet, please provide relevant information about yourself. Your answers are essential to designing an optimized individual program for you. Please answer all questions in the most accurate manner possible while being concise.

Your first assignment was to track your nutrition for one week. Please have this information ready (along with a calculator) so you can input into the form below.

MyFitnessPal Food Logs
Oh snap! Now its time to bust out the calculator. Using your MyFitnessPal food log, I need you to calculate daily averages from the week of tracking. To calculate averages, add all the daily numbers together and divide by the total number of days tracked. For example: Daily Calories = 1432 , 1656, 1980 , 1765, 1149, 2163 = 10, 145 / 6 = Average calories of 1691. Do these calculations for each category below.
Type in the average daily calories from MyFitnessPal.
Type in your average daily grams of carbohydrates from MyFitnessPal.
Type in your average daily grams of protein from MyFitnessPal.
Type in your average daily grams of fat from MyFitnessPal.
Type in your average daily grams of fiber from MyFitnessPal.
Type in your average daily grams of sugar from MyFitnessPal.
